About the Role:

The Home Health Aide plays a critical role in providing compassionate and personalized care to individuals who require assistance with daily living activities in their own homes. This position ensures the safety, comfort, and well-being of patients by delivering high-quality support tailored to their unique health needs. The aide collaborates closely with healthcare professionals and family members to implement care plans effectively and respond promptly to any changes in the patient's condition. By fostering a respectful and supportive environment, the Home Health Aide helps promote patient independence and dignity. Ultimately, this role contributes significantly to improving the quality of life for patients receiving long-term or short-term home care services.

Minimum Qualifications:

• High school diploma or equivalent.
• Completion of a state-approved Home Health Aide training program.
• Certification as a Home Health Aide where required by state law.
• Basic knowledge of patient care techniques and safety protocols.
• Ability to perform physical tasks such as lifting and assisting patients with mobility.

Preferred Qualifications:

• Previous experience in assisted living, long-term care, or home care settings.
• CPR and First Aid certification.
• Familiarity with electronic health record (EHR) systems and computer data entry.
• Strong communication skills and ability to work independently.
• Additional certifications related to patient care or geriatric support.

Responsibilities:

• Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support.
• Monitor and record patients’ vital signs and report any significant changes to healthcare supervisors.
• Provide medication reminders and assist with prescribed treatments as directed by healthcare professionals.
• Maintain a clean, safe, and sanitary environment in the patient’s home to prevent infections and promote health.
• Document patient care activities accurately using computer data entry systems and maintain confidentiality of patient information.
• Prioritize tasks effectively to manage multiple patient needs and emergencies with minimal supervision.
• Offer emotional support and companionship to patients, fostering a positive and encouraging atmosphere.
• Communicate regularly with family members and healthcare providers to coordinate care and update on patient status.

The required skills such as patient care, assisted living, and long-term care are essential for delivering hands-on support and ensuring patient comfort and safety daily. Priority setting enables the aide to manage multiple responsibilities efficiently, especially when working with minimal supervision. Shifts will be 3 hours in morning and 3 hours in the evening
