About the Role:

As an LPN, you will be responsible for providing high-quality, patient-centered care to our residents. Your duties will include:

• Providing direct patient care , including administering medications, performing vital signs, and assisting with daily living activities

• Observing and reporting changes in residents' conditions to ensure prompt and effective interventions

• Collaborating with healthcare team members , including physicians, therapists, and other healthcare professionals, to develop and implement personalized care plans

• Maintaining accurate and detailed records of patient care and treatment

Responsibilities:

• Deliver exceptional patient care, prioritizing individualized needs and preferences

• Implement care plans in accordance with professional standards and facility policies

• Develop and maintain positive relationships with residents, families, and colleagues

• Participate in regular quality improvement initiatives to enhance patient outcomes and satisfaction

• Maintain a clean, safe, and organized work environment

Requirements:

• Current LPN licensure in the state of Ohio

• High school diploma or equivalent required; diploma or degree in nursing or related field preferred
