- Assess patients’ needs and determine appropriate case management plans within an outpatient care framework.
- Lead and coordinate the interdisciplinary team to implement the plan of care and monitor progress toward goals.
- Communicate with patients and families to educate about care options, medications, and symptom management.
- Facilitate transitions between care settings and coordinate referrals to community resources and services.
- Maintain accurate documentation in the patient’s chart and ensure timely follow-up and accountability.
- Apply clinical judgment to symptom assessment and escalation when necessary, prioritizing patient safety and quality of life.
- Active RN license in the state of Colorado
- Hospice experience required
- Educational background: RN degree (accredited program)
- Ability to work in an outpatient care environment and manage a caseload with strong assessment and critical thinking skills
- Comfortable coordinating with an interdisciplinary team and documenting care plans in the charting system
- Eligible to work in the United States
- Guaranteed 40 hours per week and a 13-week contract
