Responsibilities

• Assess patients’ needs and determine appropriate case management plans within an outpatient care framework.
• Lead and coordinate the interdisciplinary team to implement the plan of care and monitor progress toward goals.
• Communicate with patients and families to educate about care options, medications, and symptom management.
• Facilitate transitions between care settings and coordinate referrals to community resources and services.
• Maintain accurate documentation in the patient’s chart and ensure timely follow-up and accountability.
• Apply clinical judgment to symptom assessment and escalation when necessary, prioritizing patient safety and quality of life.

Requirements

• Active RN license in the state of Colorado
• Hospice experience required
• Educational background: RN degree (accredited program)
• Ability to work in an outpatient care environment and manage a caseload with strong assessment and critical thinking skills
• Comfortable coordinating with an interdisciplinary team and documenting care plans in the charting system
• Eligible to work in the United States
