Physical Therapist

Physical Therapist (PT) – Kettering Home Care. Full‑time position in Dayton/West Carrollton (45439) and Kettering (45429). Work schedule: Monday—Friday 8:30am—5:00pm; weekend rotation approximately every 12 weeks; one holiday per year. Compensation & Benefits Competitive base salary plus uncapped bonus structure. Responsibilities Patient Care & Treatment : Provides skilled physical therapy in the patient’s home as directed by the attending physician, relieves pain, restores function, maintains performance using exercise, massage, heat, water, etc.; documents patient responses, changes, and progress; helps reduce rehospitalizations by front‑loading visits for high‑risk patients and teaching preventive measures; arranges for outpatient services when home care is not possible. Planning & Assessment : Performs OASIS assessments to develop individualized plans of care; completes initial evaluation and admission visit within 48–72hours of referral; evaluates and adjusts the Plan of Care based on patient response to treatment. Family & Team Communication : Counsels and instructs patient/family on disease/injury management and equipment use (wheelchairs, walkers, etc.); ensures patient/family understand financial responsibility for equipment via the DME department/MSW; oversees and supervises therapy services provided by PTAs and nurse assistants; communicates with schedulers, PTAs, and nurse aides regarding results and changes. Compliance & Administration : Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines; documents all aspects of treatment, assessments, and patient education; participates in on‑call requirements, case conferences, and mandatory meetings; maintains a professional appearance and practices agency policies. Essential Functions : Drives to the patient’s primary location per scheduled visit; attends daily visit locations; completes documentation: subsequent, discharge, eval/recert/resumption of care visits within 24hours; start‑of‑care visits within 48hours; establishes and follows plan of care as permitted within the scope of practice. Qualifications & Attributes Licensed Physical Therapist with a current license in the state of employment. Minimum of one year of experience as a PT in an acute setting required; home‑care experience desired. Strong communication skills and ability to create positive impressions. Knowledge of best practices in therapy. Ability to make appropriate physical‑therapy judgments and handle situations with the best possible solution. Detail‑oriented, observant, disciplined, and able to prioritize. Valid driver’s license and auto insurance with your name as a listed driver. Health & Safety Requirements Must meet all physical position requirements. Evidence required of a current negative tuberculin skin test or chest x‑ray. Must be free of conditions that restrict ability to perform job functions; risk for exposure to blood‑borne pathogens exists. Physical demands: constant reading/writing/speaking English; computer/PDA usage; hearing/seeing; talking, stretching/reaching, walking, sitting, bending, standing; frequent driving; lifting up to 50lbs.
