LVN / LPN - LTC/SNF

Call for Salary Rate –

Job Summary:
Seeking a licensed vocational nurse (LVN) or licensed practical nurse (LPN) to provide quality nursing care in a long-term care and skilled nursing facility setting. The ideal candidate will ensure patient comfort, safety, and well-being through proficient clinical skills and bedside care.

Responsibilities:

• Administer medications and treatments following physician orders and facility protocols

• Monitor patient vital signs and record observations accurately

• Assist with activities of daily living and basic patient hygiene

• Collaborate with interdisciplinary team to develop and update care plans

• Maintain comprehensive documentation of patient care activities

• Ensure infection control procedures and safety standards are consistently upheld

Qualifications:

• Valid and current LVN or LPN license issued by the state of New Hampshire

• Proven experience in long-term care or skilled nursing facilities

• Ability to accurately assess patient conditions and respond appropriately

• Strong interpersonal and communication skills

• Knowledge of state and federal healthcare regulations and compliance standards

• Commitment to patient confidentiality and professional ethics
