Responsibilities: Assess home health patients in stable and emergent situations to identify physical, psychosocial, and environmental needs. Develop, document, and evaluate the Plan of Care to ensure quality and continuity of care. Educate patients and families on care, medications, and home safety. Supervise and provide clinical directions to home health staff to ensure quality service. Coordinate referrals to appropriate agencies and ensure compliance with third-party requirements.
