Job Details: Job Location: Jammes Road - Jacksonville, FL 32210, Position Type: Full Time, POSITION OBJECTIVES: Provide direct medical care and support to residents living in a community home setting consistent with requirements of The Agency for Persons with Disabilities (APD) standards. Focus on routine health maintenance, behavioral understanding, and long-term advocacy. Administer medications and monitor the health and safety of residents with developmental disabilities, mental health conditions and illnesses. POSITIONS SUPERVISED: None RESPONSIBLE TO: Vice President, Residential Services SPECIFIC FUNCTIONS Medication Administration: Securely manage, order, and administer routine and psychotropic medications, carefully tracking adherence and potential side effects. Review MARs, medication records and medication changes for accuracy. Work to ensure that medication errors are prevented. Health and Behavioral Monitoring: Observe residents for subtle physical or behavioral changes, especially for individuals who are non-verbal and communicate distress through behavioral shifts. Follow up on medical appointments, hospital discharges and treatment recommendations. Seizure and Feeding Management: Monitor and care for residents with specialized needs, including seizure protocols, G-tube feedings, or modified diet textures (mechanical soft/pureed). Direct Support Collaboration: Partner with Direct Support Professionals (DSPs) to ensure individual care plans (ICPs) are properly followed during daily routines. Regulatory Compliance: Maintain rigorous documentation adhering to state agency regulations (such as APD or similar state IDD oversight boards) regarding medical logs and incident reporting. Resident Advocacy: Accompany residents to medical appointments to accurately communicate health histories and advocate for their specialized care needs. Support Plan Meetings: Participate in support plan meetings, following person-centered planning principles and ensuring that each individual’s choices and independence dictate their care. Qualifications: Qualifications & Requirements Licensure: Active and unencumbered LPN license. Certifications: Current BLS / CPR certification. Experience: Prior experience working with individuals with IDD, autism, or in a long-term care setting is highly preferred. Special Skills: High level of patience, strong non-verbal communication interpretation, and crisis intervention/de-escalation training (e.g., CPI or MANDT) is a plus. Strong working knowledge of medication management, basic clinical assessments, and compassionate communication. Salary: $58,000 annually Please note that all potential employees must complete a background screening housed and managed by the Agency for Health Care Administration called the Florida Care Provider Background Screening Clearinghouse.
