Conducts thorough assessments of patients' goals, biophysical, psychosocial, environmental, and discharge planning needs. Acts as a liaison to post-hospital care providers and community health resources. Manages patient transitions through the system, transferring accountability upon discharge. Attends practice group meetings and participates in regular peer reviews.
