Job Location: Miami FL 33135 Candidates must reside in FL within one of the following ZIP codes: 33126, 33125, 33130, 33135, 33142

Training: Remote via Microsoft Teams (approximately 4–6 weeks)

Travel: 50–75% travel within the Mid-Miami region for in-home visits, assisted living facilities, and nursing homes.

Position Summary

We are seeking a self-motivated, energetic, detail-oriented, highly organized, and tech-savvy Bilingual Case Management Coordinator to join our Case Management team. This role is responsible for assessing, planning, implementing, and coordinating case management services through both telephonic and face-to-face interactions. The Case Management Coordinator manages a caseload of supportive and medically complex members, ensuring appropriate healthcare services, care coordination, and improved health outcomes.

Required Qualifications

Fluent Bilingual Spanish/English (reading, writing, and speaking) Bachelor's degree required Social Work or related field preferred No nursing degrees Minimum 1 year of Case Management experience Ability to multitask, prioritize, and adapt in a fast-paced environment Strong verbal and written communication skills Proficient with Microsoft Office (Word, Excel, Outlook) Must reside within the required Mid-Miami ZIP codes

Experience Requirements

Minimum 1 year of Case Management experience (Required) Case Management Certification (Preferred) Long-Term Care experience (Preferred) Managed Care experience (Preferred) Strong Microsoft Office skills, including Excel Fluent in Spanish and English

Education

Bachelor's Degree required Social Work or related field preferred Nursing degrees are not eligible
