Case Manager-Corporate

Location: Corporate Office, Commerce Ca, 90040

Position Purpose: The Case Manager will provide comprehensive case management services to patients of JWCH, Wesley Health Centers and those referred from the Kaiser Emergency Department (ED) program. This role involves assessing patient needs, developing care plans, coordinating services, and advocating for patients to ensure they receive the necessary care and support. The Case Manager will work collaboratively with healthcare providers, social services, and community resources to improve patient outcomes and enhance the quality of care.

Principle Responsibilities:

1. Conducts comprehensive case management services, including intake and assessment, development of a comprehensive care plan, implementation of the care plan, specialty referrals, and follow-up.
2. Monitor and adjust care plans as needed to ensure optimal patient outcomes.
3. Connects clients to support services and provides appropriate referrals including but not limited to medical, addiction medicine and recovery programs, dental, transportation and housing.
4. Ensure timely follow-up on referrals and appointments.
5. Collaborate with multidisciplinary team to address the needs of persons living with mental illness, substance abuse disorders, complex medical conditions, or experiencing or at risk of homelessness
6. Advocates on behalf of clients with healthcare professionals, utilizing motivational interviewing, trauma-informed care, and harm-reduction approaches
7. Coordinate a range of medical and psychosocial services to address social, emotional, and practical needs
8. Assist patients in navigating healthcare services and overcoming barriers to care.
9. Educate patients and families on available resources and support services.
10. Maintain accurate and up-to-date patient records, including assessments, care plans, and progress notes.
11. Prepare and submit required reports and documentation in a timely manner
12. Coordinate outreach activities in and outside of the clinic
13. Maintain productivity requirements to meet contract compliance.
14. Complete and maintain daily documentation to DMH contract standards in a timely manner.
15. Participate in team meetings, case conferences, and other collaborative activities.
16. Communicate effectively with patients, families, and healthcare team members.
17. Participate in the development and implementation of case management programs and initiatives.
18. Adhere to all organizational safety policies, procedures, and regulatory requirements to ensure a safe environment for patients, staff, and visitors.
19. Participate in required safety and compliance trainings and apply learned practices in daily work activities.
20. Identify, report, and help mitigate unsafe conditions, incidents, or hazards in the workplace.
21. Use equipment, tools, and supplies safely and responsibly in accordance with organizational and regulatory standards.
22. Maintain awareness of infection control, occupational health, and emergency preparedness procedures appropriate to the role.
23. Participate in JWCH’s customer service training called AIDET on an annual basis.
    • Demonstrate an understanding of AIDET (Acknowledge, Introduce, Duration, Explanation, and Thank You) to apply in patient encounters or team discussions.
    • Utilize the AIDET communication framework to communicate with patients and team members in a manner that reduces patient anxiety, increases patient compliance, and improves clinical outcomes.
24. Perform other duties as assigned.

Requirements:

1. AA or Bachelor’s degree in Social Work, Sociology, or a related field, with at least 1 year of experience as a care coordinator, community health worker, or in a similar role preferred
2. High School Diploma or GED, with at least 3-4 years of experience as a care coordinator, housing navigator, community health worker, or in a similar role.
3. Willing and able to work in low-income areas of Los Angeles with persons experiencing mental illness and are homeless or at risk of homelessness.
4. Strong assessment and care planning skills.
5. Excellent organizational and time-management skills.
6. Effective communication and interpersonal skills.
7. Experience with Electronic Health Recording (EHR), Next-Gen.
8. Bachelor’s degree in social work or equivalent experience is preferred.
9. Bilingual (English/Spanish) ability is preferred.
10. Experience working in a DMH funded program is preferred.
