Title: Licensed Practical Nurse (LPN/LVN) - Outpatient Clinic

Location: Medford, MA 02155

Pay: $37.17 per hour

Schedule: 2-3 weekdays plus every other weekend; shifts are typically 7-10 hours within clinic operating hours, with some major holiday coverage required. Work schedules are created in 6-week blocks following completion of the 3-week training and orientation period.

Employment Type: W2 Contract; temporary assignment with potential for extension or conversion to FTE based on business needs, but conversion is not guaranteed

Start Date: Tentatively September 14, 2026

Assignment Length: Expected through March 2027

Licensed Practical Nurse Position Summary

A healthcare organization is seeking a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to support patient care in an outpatient clinic setting. In this role, you will use your clinical knowledge and hands-on nursing experience to assess patient needs, provide treatments and medications, monitor patient conditions, document care, and collaborate with other healthcare professionals. You will play an important role in supporting patient health, coordinating access to care, and helping patients manage their medications, self-care needs, and health conditions.

Licensed Practical Nurse Responsibilities

• Administer oral, injectable, intravenous, and topical medications.
• Evaluate patient conditions, monitor pain levels, and document treatment responses.
• Monitor and accurately record vital signs, including blood pressure, heart rate, respiratory rate, and temperature, and identify abnormalities.
• Perform routine patient assessments to monitor health and overall well-being.
• Help manage patient access to healthcare and connect patients with community resources related to medication administration, self-care techniques, and disease management.
• Conduct medication reviews to identify potential side effects and drug interactions.
• Perform mental health assessments to screen for depression and evaluate mood, behavior, and cognitive function.
• Accurately document patient medical history, including illnesses, medications, allergies, surgeries, and family history, using electronic health records.
• Communicate effectively with patients, families, and other healthcare professionals.
• Maintain an organized, well-stocked, clean, and safe clinic environment.
• Collaborate with members of the clinical team to support quality patient care and positive health outcomes.

Licensed Practical Nurse Qualifications

• Must hold a full, active, and unrestricted license as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
• Strong clinical assessment, communication, documentation, and patient care skills.
• Ability to work effectively as part of an interdisciplinary healthcare team.

Licensed Practical Nurse Preferred Qualifications

• Experience working in a Primary Care, Urgent Care, or Emergency Room setting.
• Experience using Epic EMR or similar Electronic Medical Record systems.
• Experience with electronic health records and clinical documentation.
