Details Home Health department serving patients in Port Angeles and Sequim Post-acute care setting providing nursing visits to patients in their place of residence Functions as Visit RN, delivering care as delegated by the Case Manager RN who oversees overall care planning Full time schedule at 0.9 FTE, approximately 36 hours per week Day shift schedule with 5x8 shifts Weekend and holiday coverage required 13-week assignment length Uses EPIC EMR Requirements Active WA State RN license or compact license required Current BLS certification required Minimum 1 year of recent experience in home health, acute, or long-term care required Associate's degree required, BSN preferred Valid Washington driver's license required Must have reliable personal automobile and proof of insurance for work-related travel Driving record must comply with facility standards Must have a cell phone with activated voicemail Must have computer internet connectivity for work Additional Information Provide nursing visits for patients as part of an interdisciplinary home health team Communicate patient progress, recommended actions, and physician orders to the Case Manager RN Apply full working knowledge of nursing principles and practices to make independent decisions in complex situations under general supervision Travel to multiple patient residences to deliver care as prescribed by physicians American Traveler Job ID P-747605.
