Intake Registered Nurse (RN)

Location: Wichita, KS
Job Type: Full-Time
Schedule: Monday-Friday, 9:00 AM-5:00 PM
Weekends/Nights: No weekends or nights
Setting: Non-medical home health; house visits for Medicaid patients

Education & Licensure

• Associate's Degree in Nursing (ADN) required.

• BSN preferred.

• Active RN license in good standing with the Kansas State Board of Nursing.

• No specific certifications required.

Requirements

• Minimum 2 years of RN experience ; 5 years preferred.

• 1-2 years of intake, assessment, triage, or patient coordination experience preferred.

• Home health or community-based care experience preferred.

• New graduates are not preferred ; ideally 2-3 years of experience.

• Strong communication and clinical assessment skills.

• Good critical-thinking and decision-making skills.

• Strong organizational and time-management skills.

• Self-motivated, dependable, and able to take initiative .

• Compassionate and patient-centered approach.

• Ability to maintain HIPAA compliance and confidentiality .

• Able to manage multiple tasks in a fast-paced environment .

• Strong teamwork, resilience, follow-through, and attention to detail .

Key Responsibilities

Clinical

• Complete initial patient assessments within 48 hours of assignment.

• Educate patients and families on home health care.

• Visit approximately 4-8 patients per day .

Intake Support

• Work with the Intake Coordinator on patient admissions.

• Review referrals and clinical documentation.

• Verify required documents and physician orders.

• Communicate with patients, families, and referral sources.

• Provide clinical input on patient care needs.

• Coordinate scheduling and start of services.

• Ensure intake documentation is accurate and compliant.

• Report clinical concerns or admission barriers to leadership.

Administrative

• Follow agency policies and Kansas state regulations .

• Protect patient rights and confidentiality.

• Maintain HIPAA compliance .

• Complete documentation accurately and on time.
