RN Case Manager: Care Coordination & Discharge Planning

Lifepoint Health® is seeking an RN Case Manager in Florence, Alabama, responsible for monitoring and managing patient care to ensure optimal outcomes and patient satisfaction. This role necessitates critical thinking and decisive judgment, working collaboratively with the multidisciplinary team. Applicants must have an RN licensure in Kentucky, with a BSN preferred, and are required to obtain ACM certification within two years.
