Responsibilities Provide direct patient care including wound care, medication administration, and vital sign monitoring Develop and follow individualized patient care plans in coordination with supervising RNs and physicians Observe and document patient conditions, reporting any changes to the appropriate healthcare team members Assist patients with activities of daily living (ADLs) and personal hygiene as needed Educate patients and family members on care procedures, medications, and health management Maintain accurate and timely clinical documentation and patient records Ensure compliance with all state, federal, and agency healthcare regulations and standards Requirements Active Licensed Practical Nurse (LPN) license in the state of Georgia Previous home health, long-term care, or clinical nursing experience preferred Strong knowledge of nursing procedures, medical terminology, and patient care best practices Excellent communication and interpersonal skills with patients, families, and care teams Reliable transportation and valid driver's license for home visit travel in the Canton, GA area CPR/BLS certification required or ability to obtain upon hire Compassionate, patient, and detail-oriented with a commitment to quality care About Us Presidential Home Care LLC is a trusted home healthcare provider serving the Canton, GA community and surrounding areas, dedicated to delivering exceptional, dignified care to patients in the comfort of their own homes.
