Case Manager: Housing & Social Justice Advocate
PATH (People Assisting the Homeless)
PATH (People Assisting the Homeless) is seeking a Case Manager III to support clients through all phases of homelessness, from interim housing to permanent housing, at the Yale Navigation Center in California. You will conduct intake, develop ISP goals, coordinate referrals, maintain confidential records, and ensure HIPAA compliance while engaging with diverse clients and collaborating with community partners. #J-18808-Ljbffr PATH (People Assisting the Homeless)
