Key Responsibilities\n- \n
- Conduct thorough psychosocial assessments through interviews and observations, as well as the review of medical records, to identify emotional, social, and environmental factors affecting patients' experiences. \n
- Provide empathetic counseling and emotional support to patients and their families, navigating complex feelings of grief, loss, and life transitions. \n
- Collaborate with physicians, nurses, and other healthcare professionals to create tailored treatment plans that incorporate psychosocial needs and foster holistic care. \n
- Assist patients and families in identifying and accessing community resources, support groups, and services designed to improve their overall quality of life. \n
- Educate families about care options and community resources available post-rehabilitation to ensure ongoing support. \n
- Actively participate in discharge planning to ensure seamless transitions from hospice care, facilitating connections to other necessary care settings. \n
You will primarily work with patients in a hospice setting, which typically includes individuals with advanced illnesses who require specialized psychological support and assistance in managing end-of-life issues. Your role is crucial in enhancing the emotional well-being of both patients and their families, ensuring that care is compassionate and individualized.
Qualifications\n- \n
- A Master’s degree in Social Work from an accredited program. \n
- Active state licensure as a Licensed Clinical Social Worker (LCSW). \n
- Strong communication, interpersonal, and therapeutic skills to effectively engage with patients and their families. \n
- A deep understanding of hospice care principles and end-of-life issues. \n
- Ability to work compassionately with individuals experiencing grief and loss. \n
