Responsibilities

• Perform comprehensive occupational therapy evaluations for residents in the skilled care setting.
• Develop and implement individualized treatment plans to improve activities of daily living, dexterity, upper-limb function, and mobility.
• Deliver direct patient care using evidence-based interventions, assistive devices, and adaptive equipment as appropriate.
• Monitor patient progress, document outcomes, and update plan of care in the medical record.
• Collaborate with nursing, therapy, and social work teams to optimize rehabilitation outcomes and plan transitions or discharges.
• Educate residents and families on home exercise programs, safety strategies, and ongoing treatment goals.

Requirements

• Ability to work effectively in a Skilled Nursing Facility setting.
• Availability for a 13-week travel assignment with guaranteed hours.
• Willingness to work a 5x8 day schedule.
• Strong communication and documentation skills; demonstrated ability to collaborate with a multidisciplinary team.
