What You’ll Do:

- Build relationships with members through monthly communication and ongoing follow-up

- Review person-centered service plans with members and their families

- Coordinate covered and non-covered benefits and services

- Collaborate with families, providers, and vendor agencies to ensure continuity of care

- Identify member needs and make sure concerns are addressed promptly

- Maintain timely and thorough documentation in member charts

- Participate in weekly interdisciplinary team meetings

What We’re Looking For:

- Fluency in Mandarin or Cantonese is REQUIRED

- Strong communication and relationship-building skills

- Knowledge of member assessments and clinical review procedures

- Ability to work effectively with members from diverse backgrounds and age groups

- Organized, compassionate, and comfortable coordinating multiple aspects of member care
