Benefits after 90 days offered: Medical, Dental, Vision, Life Insurance, Sick timeAfter One year of employment: 401K, Vacation timeGroup Home ManagerResponsibilitiesLeading and participating in daily activities for the designated program regarding various servicesDevelop and oversee the monthly calendar and staff rotation schedule in coordination with Group Home Director and Program DirectorLead, train, and coach direct support professionals under the direction of the Group Home Director and program directorRecommend, implement, and evaluate programs and goals to ensure quality standards of care, self-determination practices, and fiscal requirements are metProvide direct service as neededMonitor and ensure proper administration of medication along with proper documentationAssist in building a support network between the individual and the communitySupport and encourage individuals to increase their ability to be as independent as possibleFollow all procedures and policies for paperwork and mandatory reportingProvide guidance and support to all staff membersProvide for the safety, emotional and physical needs of all individualsCoordinate to make sure that all information is received, and supplies are available prior to receiving a new individualMaintain petty cashMaintain client fundsUtilize means for furthering professional growth by attending mandatory staff meetings/trainingsEnsure all documentation required for individuals care, including, but not limited to incident reports, daily assessmentsCommunicate with families and Support Coordinators as neededAttend Annual ISP’s and (180-day meetings if needed)Make new client files, update information as needed (vitals, consents etc.)Oversee transportation for community clients and outingsOversee cleanliness and maintenance of AZDT vansTransport clients safely to and from various locations, depending on activity, ranging in time from approximately 30 minutes - 3 hours per day or as neededMonthly MPR’s (complete, print, and give to Director by the 7th of every month)Daily staff attendance calendarAll Staff “Absence Request”Monitor staff overtimeComplete IR’s, review, and e-mail to DirectorDrives vehicles following the driving policies and ensures van information is currentMust attend all training sessions required by the companyMaintain active certifications to all classes neededKeeping a positive attitudeCreates new and innovative service models for people with disabilities based on individual choicePhysically able to be on your feet for most of your shiftRequirementsMust have a VALID, Level 1 fingerprint clearance cardMust have a valid AZ Driver’s LicenseBachelors degree in related health field and/or 1 or more years’ experience working with DDD members as a DCWMust be at least 18 years of age or olderAbility to pass a background check and drug test where applicableAbility to speak, read and write English proficientlyAbility to learn prevention and support methods and implement themAbility to learn how to administer medicationAbility to pass all required trainingAbility to lift 50lbs and perform physical activities #J-18808-Ljbffr Arizona Dream Team 1 Llc
