divh2LPN Coordinator/h2pReport To: Registered Nurse - (RN)/ppDepartment: Nursing Department/ppFSLA Status: Exempt - Hourly/ppRevision Date: 1/01/2019/ppSummary:/ppThis position is primarily responsible for managing the care of patients, promoting effective education, self-management support, and timely health care delivery by performing the following duties./ppEssential Duties And Responsibilities:/ppCore duties and responsibilities include the following. Other duties may be assigned./ppServe as liaison between resident, family, physician, and other health care providers regarding health care issues./ppMaintains a current knowledge of OSHA and state regulations and routinely monitors community compliance with regulations related to resident care./ppProvides follow-up contact with patients to ensure compliance with recommendations./ppEducates and involves patients to improve their health-patient engagement./ppAssists patients to set goals, progress and identify barriers when patients are not meeting treatment goals./ppManages many aspects of patient care: referrals, hospitalizations, ER visits, ancillary testing, and other as directed to achieve optimum outcomes in a safe and cost-effective manner./ppDevelops a list of medical supply and community resources available to patients./ppOperates department(s) within established budgetary guidelines./ppCoordinates continuity of care with the patient and or family to ensure compliance/ppProvides clinical follow-up with patients, giving information and guidance to enhance patient-care team communication/ppMaintains accurate and timely documentation/ppSupervisory Responsibilities:/ppThis job has no supervisory responsibilities./ppRequirements/ppQualifications:/ppTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
