Daily Communication Log-initial after last entry read (you may remember the last date you were there, but do you remember if you had a chance to read the log then?)
RN Review Binder-Ensure all appointments on the calendar since your last review have Consults in the RN Review Binder. Check all med changes, D/C, new meds on MAR correctly, labels correct, and old meds in discard box. Check ALL renewal Rx against MAR entry
Prepare Outgoing Consults-VS, Menses, seizurecheck against calendar for upcoming week
Write Nursing Progress Notes-note PRN use, health issues being monitored, overall health, medication effect. File-Ensure Manager has reviewed all the consults or signed the Medical Appointments Week offor that week and then file completed consults in the Individual's medical record
Reminders: When notified of med changeensure it arrives, plan to red/black dot later and remove old labels
****Also answer triage calls, follow-up medication refills, in addition to fulfilling the Weekly/Monthly/As Due responsibilities
Weekly (Initial all documents weekly in red)
- Medication Inventory-Refills
- MAR- note and address -missing signatures -new/changed medications (check Rxs to see started on time, entry correct, label correct, old meds in discard box) -refusals -comments on back of MAR -PRN use -ensure each signature for B/P, P, wt, FS, etc. Has documented values
- Empty Blisterpacks-ensure all doses accounted for
- Controlled Counts-do a count yourself and ensure it is correct
- Dispose Medications within 72 hours if controlled, within a week if not
- Health Stats-VS in range, FS in range, seizures, weights, Menses, Bowel charts, Glucometer memory-check against FS log
