Are you the person hospital case managers call when they need a solution?

Champion Care is hiring two experienced Hospital Liaisons to expand our presence throughout southeastern Wisconsin. One position will focus on Milwaukee County, and the second will focus on Waukesha County.

This is not a desk job. You will be in the market building relationships, creating opportunities, removing barriers to placement, and connecting patients with the skilled nursing and rehabilitation services they need.

The Opportunity

As a Hospital Liaison, you will become a trusted resource for hospital case managers, discharge planners, physicians, social workers, patients, and families. You will represent a growing network of skilled nursing and rehabilitation centers while partnering closely with our facility leadership and admissions teams.

Your success will be measured by the relationships you build, the opportunities you uncover, and the patients you help transition successfully into the next level of care.

What You Will Do

• Build and strengthen referral relationships throughout your assigned county

• Maintain a consistent presence in hospitals and other healthcare settings

• Promote our skilled nursing, rehabilitation, long-term care, and post-acute services

• Identify referral opportunities and help move admissions forward quickly

• Work directly with facility teams to overcome barriers and coordinate smooth transitions

• Educate referral sources about our clinical capabilities and available services

• Develop market strategies that support admissions and sustainable census growth

• Track referral activity, competitive trends, and new business opportunities

• Represent Champion Care at professional, healthcare, and community events

Who Will Thrive Here

You are confident walking into a hospital, starting conversations, and developing relationships that lead to results. You understand that great business development is built through credibility, responsiveness, visibility, and follow-through.

Previous experience in healthcare sales, hospital liaison work, admissions, case management, discharge planning, physician relations, or post-acute care is preferred. Skilled nursing experience and established relationships within Milwaukee or Waukesha County would be a major advantage.

An RN or LPN license is preferred but not required.
