Overview Caring Hands Loving Hearts (CHLH) is a Pennsylvania ODP-authorized provider of Home and Community-Based Services for individuals with intellectual disability, developmental disability, and autism. The organization supports young adults and adults with ODP Waiver Funding, delivering services in homes and community settings where daily life occurs. CHLH offers in-home and community support, companion services, respite care, and shift nursing provided by both RNs and LPNs. Role Description The Licensed Practical Nurse (LPN) is a full-time, on-site role based in Jamison, PA, providing nursing care to individuals with intellectual and developmental disabilities and autism. The LPN will deliver shift nursing services, including monitoring health status, administering medications and treatments, documenting care, and supporting individuals with activities of daily living as needed. This role involves implementing physician orders and care plans, collaborating with RNs, Program Specialists, Direct Support Professionals, and families to ensure person-centered care. The LPN will observe and report changes in health or behavior, assist with preventative and routine health care, and help maintain a safe, respectful environment aligned with PA ODP’s Everyday Lives principles. The nurse will also participate in team meetings, follow all regulatory and agency policies, and contribute to continuous quality improvement. Qualifications Current, unencumbered Licensed Practical Nurse (LPN) license in the Commonwealth of Pennsylvania. Demonstrated clinical skills in medication administration, basic assessments, documentation, and infection control. Experience or strong interest in working with individuals with intellectual and developmental disabilities and autism. Ability to follow person-centered care plans and collaborate effectively with interdisciplinary teams, individuals, and families. Strong communication, observation, and critical-thinking skills with attention to detail and accurate recordkeeping. Basic computer skills for electronic documentation and communication; ability to learn agency systems. CPR/First Aid certification (or willingness to obtain), valid driver’s license, and ability to meet any required background checks.
