Physician Assistant

Signify Health seeks a Physician Assistant to deliver comprehensive in-home and virtual evaluations for vulnerable patients, supporting value-based care programs. You will perform detailed health assessments, manage chronic conditions, update and interpret EHR data, and collaborate with interdisciplinary teams to close care gaps and coordinate follow-up services. Working within a mission-driven, data-informed environment, you’ll use modern technology and analytics to inform care decisions, educate patients and caregivers, and contribute to improved outcomes and reduced healthcare costs while maintaining a healthy work–life balance.
