Job Overview CareBuilders at Home is seeking a compassionate and experienced Registered Nurse (RN) to conduct in-home assessments for clients who require support services in the home. Duties Conduct onboarding in-home assessments of clients seeking support services at home Create non-medical care plans to include activities of daily living, light housekeeping, personal care, medication reminders, community transportation, respite care, etc. Onboard new clients and periodically reassess client's progress with the in-home care services Educate patients and their families on non-medical in-home care services and scheduling Utilize electronic medical record systems (AxisCare) to accurately document patient information Schedule initial caregiver visits Requirements Pass drug screening Valid Driver's License Own reliable transportation to travel throughout Prosper and surrounding areas Proof of car insurance Social Security Card or Birth Certificate Negative TB skin test Provide three work related references Job Type Part-time Schedule Varies based on appointments Work Location On the road #J-18808-Ljbffr CareBuilders At Home, Inc.
