Hospice Admission Nurse

Overview Hartford Healthcare is seeking a compassionate Hospice Admission Nurse to complete timely home and facility-based hospice admissions. You will conduct comprehensive assessments, explain hospice services, obtain consents, and develop initial care plans in collaboration with an interdisciplinary team. Responsibilities Conduct comprehensive assessments for hospice admissions in home and facility-based settings. Explain hospice services, obtain informed consent, and develop initial care plans with the interdisciplinary team. Coordinate with physicians, case managers, and families to ensure a smooth transition to hospice. Ensure accurate documentation in the electronic health record (EHR) and regulatory compliance.
