We foster a supportive culture where every team member is empowered to grow, contribute, and make a meaningful impact on the lives of patients, families, and the communities we serve.Location: Stockton and surrounding areas (Stockton County)Schedule: Monday- Friday with weekend rotationsWhat We Offer:$10,000 Sign-On Bonus, paid in installments at 3, 6, 9, and 12 months of employmentCompany vehicle and fuel cardCompetitive salaryComprehensive benefits package, including:Medical, dental, and vision insuranceLife insuranceAD&D insuranceShort- and long-term disability coverageEmployee Assistance Program (EAP)401(k) with a 4% company matchOngoing professional development and career advancement opportunitiesA collaborative, mission-driven team committed to delivering exceptional patient careResponsibilitiesProviding treatment as ordered by the attending physician, dentist, or podiatrist.Assisting in developing and updating the plan of treatment or Plan of Care (POC).Assisting the physician in evaluating level of function.Observing, recording, and reporting information on the patient's condition to the attending physician, case manager, and in the patient's health record.Advising, consulting, and when appropriate, instructing family and other LORIAN personnel in patient's therapy program.Teaching and supervising other healthcare personnel when appropriate.Evaluating the home environment and making appropriate recommendations.Participating in in-service education programs.QualificationsMinimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.Graduate of a physical therapy school approved by the Council of Medical Education of the AMA in Association or its equivalent.Licensed or registered to practice as a PT in the state of practice.Has the ability to read and follow written instructions and document care given.Is self-directing and has the ability to work with little direct supervision.Possesses and maintains current CPR certification.Is a licensed driver and has an automobile that is insured in accordance with Lorian Health (LORIAN) requirements.Must be able to verbally communicate in the language of the client and document all client-related activity practices.This position is based on a pay per visit compensation model.
