This is not bedside nursing, traditional therapy, home health, or facility case management. It is an autonomous, relationship-based role where you assess needs, solve problems, coordinate care, advocate for clients, and help families make complicated decisions.

What Is Aging Life Care Management?

Many excellent healthcare professionals have never heard of Aging Life Care Management—and that’s okay.

An Aging Life Care Manager serves as the central point of coordination for an older adult and their family. Instead of addressing just one episode of care, you look at the whole person and the whole situation.

What Will You Actually Do?

Then we put your strong responsibilities list here: comprehensive assessments, plans of care, appointments, advocacy, care coordination, family communication, monitoring changes, evaluating living arrangements, preventing crises, etc.

Your Professional Background Could Be a Great Fit

We are interested in experienced:

Registered Nurses (RN)

Social Workers

Occupational Therapists (OT)

Physical Therapists (PT)

Speech-Language Pathologists (SLP)

You do not need previous Aging Life Care Management experience.

What matters is that you bring a strong professional foundation, understand complex client needs, communicate well with families and providers, and can think beyond the boundaries of your individual discipline.

We Will Teach You Aging Life Care Management

This is an important selling point, so I wouldn't bury it.

We will train the right professional in our approach to comprehensive assessment, care planning, advocacy, documentation, family communication, and ongoing care management.

• You want to use more of your professional knowledge than your current role allows.

What Your Day Might Look Like

This is one I would add. It makes an unfamiliar profession much easier to imagine.

One morning you may visit a client at home to assess why they are suddenly struggling with medications. Later, you may accompany another client to a physician appointment, talk with an adult daughter about increasing care needs, coordinate services with a home-care agency, or visit an assisted living community to determine whether it can meet a client's needs.

You're not simply making recommendations—you follow the situation, coordinate the pieces, and help make sure the plan actually works.
