Job Description

We are seeking an experienced RN Case Manager for our Home Health division. The ideal candidate will join our South team and will be responsible for managing a caseload of 5-6 patients per day, focusing on high-quality care and comprehensive patient documentation.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Manage a caseload effectively, ensuring timely follow-ups and patient education.
• Coordinate with multidisciplinary teams to provide high-quality patient care.
• Maintain accurate and up-to-date patient records and documentation as per OASIS standards.
• Implement wound care procedures and assessments as needed.

Required Experience / Certifications / Licensure

• Active RN license in the state of New Jersey.
• Minimum of 2 years of experience in home health nursing.
• Strong experience in case management and OASIS documentation.
• Preferred experience in wound care management.
