Responsibilities
- Lead coordination of patient care across multiple disciplines within the care team
- Align the interdisciplinary care plan with the patient’s clinical course and post-discharge needs
- Serve as a liaison among patients, families, nurses, physicians, and other providers to optimize outcomes
- Monitor resource utilization, diagnostic testing, treatments, and procedures to ensure appropriate care
- Collaborate with inpatient case managers to assess and plan transfer and continuation of care

Requirements
- Education: Bachelor’s Degree in Nursing (BSN) preferred
- Licenses/Certifications: Registered Nurse license in the state of practice (California) required; Basic Life Support (BLS) certification required
- Experience: At least two years of recent experience in the field; inpatient case management experience preferred
- Specialization: Geriatric focus with capability to manage complex patient needs
- Work authorization: Ability to work in a hospital setting in Glendale, CA
