RN Case Manager Hospice

Job Summary:

The Hospice Registered Nurse is responsible for admitting patients to hospice care, performing comprehensive assessments of patients and their families, and developing individualized care plans. This role involves coordinating care with a multidisciplinary team, documenting observations and interventions, and participating in quality assessment and performance improvement initiatives. The Hospice Nurse conducts follow-up visits and provides skilled nursing care to patients in a home hospice setting. Responsibilities:

• Admit patients to hospice service and conduct comprehensive patient and family assessments including physical, psychosocial, environmental, safety, and developmental needs.
• Develop and implement individualized plans of care based on patient assessments.
• Document observations, interventions, and patient progress thoroughly and accurately.
• Coordinate care with interdisciplinary team members to ensure holistic patient support.
• Participate in hospice program quality assessment and performance improvement activities.
• Conduct follow-up visits as needed to monitor patient condition and adjust care plans.
• Provide skilled nursing care in the home hospice environment.

Required Skills & Certifications:

• Active Registered Nurse license in the state of practice.
• Minimum of one year of hospice nursing experience.
• Strong clinical assessment, medication administration, and treatment planning skills.
• Excellent communication and interpersonal skills for collaboration with patients, families, and healthcare teams.
• Ability to work independently in a home care setting.
• Driver's license, valid car insurance, and reliable automobile for travel within assigned territory.
• Bachelor's degree in Nursing or related field preferred.

Preferred Skills & Certifications:

• Experience with interdisciplinary team collaboration in hospice or home health care.
• Knowledge of hospice regulations and compliance standards.
• Previous case management or care coordination experience.

Special Considerations:

• Work involves travel within the San Francisco, Brisbane, Daly City, Colma, South San Francisco, Pacifica, and neighboring areas.
• Must be able to work in patients' homes and adapt to varying environments.
• Participation in quality assessment and performance improvement programs required.

Scheduling:

• Work schedule typically involves field visits during regular business hours with occasional flexibility for patient needs.
• Shift assignments may vary based on patient caseload and geographic territory.
