Skilled Nursing Facility (SNF) Physician

Western Geriatrics and Neurology | Brigade Health

Orange County, CA | Full-Time

Western Geriatrics and Neurology, part of Brigade Health, is seeking a Primary Care Physician (MD/DO) to provide post-acute and long-term care services in Skilled Nursing Facilities (SNFs) .

This role is ideal for physicians who enjoy caring for medically complex older adults , building strong relationships with facility teams, and practicing in a collaborative, value-based care model.

What You'll Do

• Provide primary and post-acute care in Skilled Nursing Facilities
• Manage admissions, follow-ups, and chronic care for geriatric patients
• Collaborate with nursing staff and interdisciplinary care teams
• Participate in care planning and quality initiatives
• Maintain timely documentation and patient communication

Qualifications

• MD or DO
• Board Certified / Board Eligible in Internal Medicine, Family Medicine, or Geriatrics
• Active state medical license (or ability to obtain)

Experience in SNF, post-acute care, geriatrics, or long-term care preferred.
