Job Description

Job Summary:
Provide skilled physical therapy services to home health patients, focusing on improving mobility, strength, and functional independence. Manage patient care plans through assessment, treatment, and documentation to ensure optimal recovery outcomes.

Responsibilities:

• Assess patients' physical conditions and develop individualized treatment plans.
• Deliver physical therapy interventions and therapies in accordance with physician orders.
• Complete and document OASIS assessments accurately for home health care compliance.
• Coordinate with healthcare teams to ensure seamless patient care transitions.
• Maintain timely communication and thorough documentation of patient progress.
• Adhere to all travel, licensure, and compliance requirements, including on-call rotation if necessary.

Qualifications:

• Hold an active CA license in hand at submission.
• Possess a minimum of 1 year of home health experience.
• Have outpatient therapy experience or solid home health background.
• Demonstrate proficiency with OASIS assessments.
• Maintain current BLS certification from AHA.
• Own reliable transportation and active driver's license.
