Key Responsibilities

• Provide occupational therapy services to patients across multiple care locations
• Evaluate patients and develop individualized treatment plans to support functional independence
• Implement therapeutic interventions in alignment with facility protocols and patient goals
• Coordinate care and communicate effectively across multiple facility settings
• Document patient progress and maintain accurate clinical records in accordance with regulatory standards

Required Qualifications

• Active Occupational Therapist (OT) license in the state of Illinois
• Graduate of an accredited Occupational Therapy program
• NBCOT certification (or eligibility)
• Ability to work across multiple facility locations as required

Preferred Experience / Skills

• Prior experience working in a multi-site or split-facility setting
• Strong organizational and time management skills to manage caseloads across locations
