Activities Assistant

Reports to : Activity Director

Assistance needed for: Sunday, Monday, Friday

Job Qualifications:

- High School Diploma or equivalent necessary

- Individualized skills in areas such as crafts, music, education or physical activities helpful

- Friendly, outgoing personality, with a warm, caring and patient attitude.

- Willingness to work as part of a team.

Essential Job Functions:

- Assists in planning and implementation of organized program for recreation

- Provides a planned resident specific, goal directed recreation program for assigned residents/patients; facilitates small group activities as assigned by the director

- Ensure that all charted recreation progress notes are informative and descriptive of the services provided and indicate the residents/patient’s response to the service. Assists in developing a written comprehensive plan of care for each resident/patient recreation program that identifies the problems/needs of the resident/patient and the goals to be accomplished for each problem/need. Follow the established plan of care in providing the daily recreation program.

- Involve the resident/patient and family members in the assessment and care planning process when possible.

- Transport residents/patients to and from recreation programs.

Experience:

· Skilled Nursing Facility: 1 year (Preferred)
