Key Responsibilities

• Conduct comprehensive patient assessments in the home setting
• Develop and implement individualized plans of care
• Deliver skilled nursing services according to physician orders and clinical standards
• Educate patients and caregivers regarding medications, treatments, and disease management
• Monitor patient progress and identify changes in condition
• Collaborate with physicians, therapists, caregivers, and interdisciplinary care teams
• Maintain accurate and timely clinical documentation
• Promote patient safety, independence, and quality of life within the home environment

Qualifications

• Active and unrestricted Registered Nurse (RN) license
• Previous Home Health experience preferred
• OASIS documentation experience strongly preferred
• Current CPR certification
• Strong assessment, clinical judgment, and critical thinking skills
• Excellent communication and patient education abilities
• Ability to work independently in a field-based setting
• Valid driver's license and dependable transportation

Pay Details: $94,000.00 to $112,000.00 per year
