Infusion Pharmacist

We are seeking an experienced Infusion Pharmacist to join our team. Previous infusion pharmacy experience is required.

Responsibilities

• Review, verify, prepare, and dispense prescription medications accurately and safely.
• Prepare and compound sterile medications and solutions following proper procedures and applicable regulations.
• Monitor patient medication compliance, drug utilization, and potential overuse or drug-related issues.
• Counsel patients on medications, including proper use, dosage, storage, side effects, and potential drug interactions.
• Maintain accurate, confidential patient records and pharmacy documentation.
• Communicate effectively with physicians, nurses, pharmacists, pharmacy interns, and other healthcare professionals regarding patient medication needs.
• Identify and resolve medication-related problems and make appropriate recommendations to healthcare providers.
• Supervise and support pharmacy technicians, clerks, interns, and other pharmacy staff.
• Maintain appropriate inventory levels of medications and medical supplies.
• Educate patients on the proper use and handling of medical devices, including blood pressure monitors and glucose meters.
• Provide additional patient care services and medication-related education as appropriate.
• Ensure all medications and pharmacy operations comply with applicable laws, regulations, and safety standards.

Requirements

• Infusion pharmacy experience – REQUIRED
• Doctor of Pharmacy (PharmD) degree required.
• Active pharmacist license in the applicable state.
• Strong knowledge of prescription medications, sterile compounding, drug interactions, and medication safety.
• Excellent communication and patient-care skills.
• Strong attention to detail and ability to work effectively in a collaborative healthcare environment.
