Community Health Worker
OHSU
Our Connected Care Community Health Workers help older adults and their families access the care they need- providing resources, care coordination, and support. Working closely with primary care and behavioral health providers, they ensure our patients’ priorities are heard and addressed. Supporting Advance Directives, medication reconciliation, fall prevention, and connecting patients to community resources.
General Requirements:
- High School Diploma or GED
- 2 years’ experience in health – or service – related field
- Valid driver’s license and clean driving record
- Bilingual in English & Spanish
- Knowledge of community resources in the Columbia River Gorge, especially those available to older adults
