Wraparound Facilitator

Job TitleThe Wraparound Facilitator will work with families of children with serious emotional disturbances by facilitating a planning process that is family centered, community-oriented, strengths-based and highly individualized. Services may be provided in the participant's home or appropriate community setting.The Facilitator will deliver intensive case management targeted at increasing the youth's natural supports and strengths, fostering resilience and recovery.The Wraparound Facilitator is responsible for accurately documenting services, facilitating Wraparound Team meetings as needed, and communicating participant progress to supervisors, team members, and the referring authority.All services are delivered in compliance with the Texas Medicaid Provider Procedures Manual, the Texas Resilience and Recovery Utilization Management Guidelines, YAP standards, and all state and federal regulations governing the delivery of service.The Wraparound Facilitator will receive training in service provision, as well as weekly supervision, and will learn a great deal about community resources, networking, mental health, and more.Qualifications/RequirementsBachelor's or Master's Degree in human services related fields, as defined by Texas Administrative Code.Must complete the Wrapround Training series.Experience in the provision of home and community-based services.Proficiency in Microsoft OfficeExperience working with an Electronic Health Record (EHR) system a plus.Strong written and verbal communication skills.CPR/First Aid Certification a plus.Behavior Management (MANDT) Certification a plus.Transportation of participants is a required, and applicant must have access to reliable transportation, possess a valid driver's license, and have current auto insurance coverage.Bilingual/Spanish speaking is a plus.Benefits AvailableMedical/PrescriptionDentalVisionShort Term DisabilityUNUM Supplemental InsuranceEmployee Assistance ProgramPet InsurancePaid Time OffHoliday Pay403(b) Retirement SavingsCompetitive Weekly PayDirect DepositYouth Advocate Programs, Inc. is an Equal Opportunity Employer.
