We are offering a great INTERNAL opportunity for a PRN ACT Case Manager for our Assertive Community Team (ACT). ACT is a highly coordinated set of services offered by a group of medical, behavioral health, peer recovery support providers and rehabilitation professionals in the community who work together as a team to meet the complex needs of individuals living with severe and persistent mental illness.

Responsibilities

Carries out rehabilitation and support functions, based on the integrated Dual Disorder Treatment model, focusing on assessment and treatment, using principles of harm reduction, motivational interviewing, and CBT, under the direction of the team leader.

Requirements

• Bachelors degree in a human services or related field.
• Registered with the Virginia Board of Counseling as a QMHP or QMHP-T
• One (1) year experience working with dually diagnosed adult consumers in a mental health setting.
• Valid Drivers License with a point balance of -6 or higher.

Preferred

• Three (3) years of experience working with dually diagnosed adult consumers in a mental health setting.
• We are looking for flexible evening hours from 5:00 pm to 7:00 pm and weekend coverage.
• Will be required to travel within the Greater Reach CSB catchment area.

PRN (As Needed) Position: This is an as-needed role with no guaranteed minimum hours. Work schedules will vary based on operational needs and may include days, evenings, weekends, or holidays.

Fingerprints, State and FBI criminal record reports, drug test, and central registry report will be required upon selection.Our Agency maintains a drug-free workplace.

Annual Salary Range

Grade H-C8

$32.00 per hour
