Role Description This full-time, on-site Licensed Practical Nurse (LPN) role is based in Mount Morris, NY. The LPN provides direct patient care, including monitoring vital signs, administering medications and treatments as ordered, and assisting with activities of daily living. Responsibilities include documenting care accurately, maintaining medical records, and communicating patient status and changes to registered nurses, physicians, and other members of the care team. The LPN supports health assessments, participates in care planning, and contributes to patient education on health maintenance and disease management. The role also involves following infection control procedures, adhering to safety and regulatory standards, and collaborating with colleagues to ensure high-quality, person-centered care. Qualifications Current New York State Licensed Practical Nurse (LPN) license in good standing and graduation from an accredited practical nursing program. Demonstrated clinical skills in basic patient care, medication administration, vital signs monitoring, and infection control practices. Ability to document care clearly and accurately in electronic or paper medical records and follow established protocols and policies. Strong communication and interpersonal skills for working with patients, families, and a multidisciplinary healthcare team. Capacity to prioritize tasks, manage time effectively, and maintain professionalism in a fast-paced care environment. Commitment to ethical practice, patient confidentiality, and adherence to state and local regulations and standards of care. Basic computer literacy; experience with electronic health record (EHR) systems is preferred. Prior experience in public health, long-term care, clinic, or community health settings is beneficial.
