Physical Therapist - Travel Contract

Minimum Qualifications

• Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Ohio Physical Therapy license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Home Health experience required.
• Experience with HCHB (Homecare Homebase) required.
• Availability to work within the assigned territory covering Perry and Morgan counties.

Preferred Qualifications

• Previous travel or per-diem home health experience preferred.
• Experience managing a caseload of point-based visits preferred.

Assignment Details

• Shift pattern: Monday through Friday, 40 hours per week.
• Shift length: 8 hours per day.
• Territory includes travel within Perry and Morgan counties.
• Points-based weekly caseload: 32 points per week.
