Key Responsibilities

• Conduct thorough evaluations of new and returning patients, which includes reviewing medical histories, performing specific assessments, and developing personalized treatment plans based on evaluation findings.
• Communicate evaluation results and the treatment strategy effectively to patients and their families, ensuring they are involved in goal-setting and rehabilitation planning.
• Ensure the implementation of evidence-based treatment protocols and regularly assess their effectiveness, adjusting plans as necessary to adapt to the patient's progress and changing healthcare landscape.
• Engage in collaborative consultations with other healthcare professionals as necessary, promptly recognizing and documenting any changes in a patient's condition that impact the treatment plan.
• Perform discharge planning and ensure proper follow-up with patients to track improvement and continuity of care, maintaining accurate and timely documentation of all patient interactions and treatment activities.

Qualifications
