Discharge Coordinator, Case Management

Provides advanced discharge coordination and clerical support for the Care Management team with an emphasis on forward-facing patient and family engagement, regulatory discharge documentation, post-acute coordination, and real-time electronic tracking. The role supports timely, compliant discharge planning and serves as an operational extension of the RN Case Manager under delegated authority.

Required Skills

• Basic to intermediate computer proficiency
• Medical terminology
• Strong verbal and written communication skills
• Ability to work across multiple electronic systems
• Critical thinking and organizational skills

Required Experience

• High School diploma or equivalent
• Previous experience in a medical setting

Preferred:

• Experience in Care Management, discharge planning, utilization review, or patient access
• Familiarity with Medicare discharge rights and post-acute care workflows
