License: NYS RN License Certifications: BLS, CPI or PMCS Certification required Must-Have: Great organizational skills, EPIC experience highly preferred Description: The Clinical Nurse is a Registered Professional Nurse who provides safe and competent quality care based on nursing theory and research to a designated group of patients and significant others. Delivers patient care by incorporating the professional practice model of Relationship Centered Care and through the application of the nursing process based on theory and evidence-based practice. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care, delegating to others as appropriate. Educates patients and caregivers about their plan of care, transition of care, promotion of health, and prevention of disease while anticipating needs and readiness to learn. Communicates effectively and professionally with patients, families, and all members of the interdisciplinary patient care team. Manages assignments within the care delivery model, demonstrating effective allocation of material resources. Role models the tenets of Relationship Centered Care through various actions including admissions, bedside reporting, and purposeful hourly rounding. Considers patients' values, preferences, cultural diversity, and expressed needs in all aspects of care. Establishes and maintains a therapeutic relationship with patients and families. Uses evidence-based practices to enhance understanding of patients' perceptions of care. Initiates service recovery when concerns arise, escalating issues as necessary. Demonstrates compassion, empathy, and active listening through body language and actions. Supports staff in utilizing evidence-based practices to improve patients' perceptions of care. Conducts purposeful hourly rounding on assigned patients addressing key care aspects. Practices safe and efficient patient-centered care, identifying and escalating barriers to improvement. Demonstrates knowledge crucial to quality and safety in the patient population served. Engages in formal and informal peer and institutional review processes. Collaborates with interdisciplinary teams to create and implement quality improvement projects and nursing research studies. Implements principles of high reliability to identify and avoid high-risk behaviors, ensuring a safe environment. Documents nursing practice accurately and thoroughly in compliance with regulations and standards. Facilitates patient throughput on a shift basis. Contributes to a supportive environment of care for clinical colleagues, patients, and families. Advances clinical competence in nursing practice and projects a professional image to colleagues. Incorporates ethical principles into decision-making for patients and families. Encourages a spirit of scholarship, inquiry, and lifelong learning. Participates in shared decision-making through nursing department committees and unit initiatives.
