Wound Care Nurse

Shift: 5x8/DAYS (8:30A-5P: Alternating Weekends), Length: 13 weeks, GT: 40, Start: 2 weeks from offer

Position Requirements: At least 2 years of home health wound care experience. All candidates must have homecare homebase/OASIS experience within the last year to be considered. Experience with Wellsky EMR and wound care would be a bonus. Experience with on call/triage. Reliable and dependable, excellent time management skills. Professional, customer service excellence, compassion and caring. Licensure in hand at time of submission. Candidates do not need to be wound care certified.

On-call Schedule and Expectations: On-call would be one to two weeks a month from 5:30pm-8:30 am weekdays. It entails phone triage, very rare does it require a home visit.

Productivity Requirement: It is a 30 pts per week for full time nurses. Our point system: 1 revisit= 1pt, SOC= 2pts, ROC = 2pts, Recert =1.5pt, Oasis discharge =1.5pt.
