Case Manager – Weekend Overnight - OutreachWork Schedule: Saturday, Sunday, Monday 11pm to 7amThe Case Manager will work within our Outreach and Housing Placement Program, providing outreach to homeless people living in or near the North Shore area of Staten Island and responding to community calls regarding outreach to other areas of Staten Island.Responsibilities:Connect with homeless people living on Staten Island. Make assessments and appropriate referrals including psych evaluations, TB and HIV testing, detox, etc., complete formal assessment whenever possible.Assists clients in filling out forms for entitlements, etc. Complete psychosocial and social history when applicable.Counsel clients leading to interim, transitional, and permanent placements.Handles all paperwork and completes accurate timely statistics as required by the program.Respond on a timely basis to calls from the community regarding homeless individuals in all areas of Staten Island.Drive and escort clients to the ESU and detox as needed using the Outreach van.Participate in weekly supervision, case conferences, and staff meetings.Qualifications Needed:HSD/GED required with 1- 2 years experience with the homeless population, Bachelor's Degree preferredExperience with the homeless, substance abuse, and mentally ill populations is neededExcellent client engagement skills are a mustMust possess a clean and valid driver's license and the ability to drive a passenger vanBilingual Spanish/English preferredMust have excellent communication skills both orally and in writing with individuals and groups in regard to complex and sensitive issuesThe physical ability to perform essential job functions is neededIn addition, the Outreach Case Manager shall perform any duties as required to meet the needs of the program and the clients we serve. #J-18808-Ljbffr Project Hospitality
