Wound, Burn and Ostomy Nurse Practitioner

Wound Care Nurse Practitioner (Part-time)- Merced - CA

Shift: Days

Multiple locations: Modesto / Merced / Madera / Sonora /San Andreas - CA

Job Summary & Responsibilities:

• Evaluate and treat both acute and chronic wounds

• Create and implement individualized care plans

• Monitor patient progress and modify treatment plans as necessary

• Document wound status and care provided for all new admissions

Education, Licensure, and Experience:

• Active California RN license and Licensed Nurse Practitioner (NP)

• Minimum of 2 years of wound care experience , with strong knowledge of treatment protocols and industry best practices

• Proficient in Electronic Medical Records (EMR) systems

• Skilled in skilled nursing facility (SNF) settings ; Wound Care Certification preferred
