Community Health Worker

Program Summary:

The Flexible Integrated Treatment (FIT) Program serves infants, children and youth (ages 0-21) with a team approach to treatment planning allowing clinicians the flexibility to assess the level and type of services needed. As treatment needs vary over time, services are tailored to fit without interruption. Equally flexible is the location of service: home, school, River Oak office sites or community setting to best accommodate the family. A wide array of treatment interventions, to include multiple evidence-based programs, are used to include individual, family and group therapy.

Job Responsibilities: (essential functions)
  • Conduct outreach and engagement activities within schools, preschools, shelters, clinics, and community settings serving high-need populations
  • Build trusting relationships with children, youth, and families experiencing behavioral health challenges or risk factors
  • Provide culturally responsive psychoeducation, support, and self-management strategies to families and caregivers
  • Administer approved early screening tools within the scope of the CHW role
  • Assist clients and families with navigating behavioral health, education, primary care, and social service systems
  • Support Medi-Cal enrollment, renewals, and eligibility processes
  • Coordinate referrals and linkages to mental health, developmental, housing, transportation, childcare, and community support services
  • Participate in multidisciplinary team meetings, outreach planning, and case consultations
  • Maintain timely and accurate documentation, reports, and service records in compliance with Agency and funding requirements
  • Attend trainings, staff meetings, and community events to support ongoing professional development and collaboration
Minimum Qualifications:
  • Ability to be certified as a Community Health Worker through the California Department of Health Care Access and Information (HCAI)
  • Strong organizational, communication, and counseling skills
